FORMS
All forms will be available in print at time of drop-off. If you wish to print and fill forms before arrival, you are welcome to, but this is not necessary.
Sewing Machine Forms
Standard Intake Form - Every sewing machine requires an intake form for contact information and inventorying parts the machine(s) arrive with.
Singer Gear Release - A certain range of older Singer sewing machines have a track record of gears failing without reason. We still work on these machines, but have a few extra agreements needed before we do.
Additional Service Authorization - In cases of parts or repair needs being found, or additional service requested after drop off, this form will be emailed with information pre-filled out for the client. It is included here only for consideration in case additional work is suspected.
